Welcome to Total Family Wellness!  As a new patient you can save time during your first appointment by completing the Patient Registration Packet prior to your visit. Simply print out the Patient Registration forms, fill in the information requested, and bring the completed form with you to your appointment. Please print the forms on white paper only.
